The Achilles is the largest and strongest tendon in the body, connecting the calf muscles to the heel bone and powering every step, jump, and push-off. When it ruptures — often during a sudden sprint or pivot in a recreational athlete — patients describe a pop and the feeling of being kicked in the back of the leg, followed by weakness and difficulty pushing off.
Not Every Achilles Rupture Needs Surgery
One of the most important points we make to patients is that not all Achilles ruptures require an operation. Some patients do very well with casting or bracing combined with Platelet-Rich Plasma (PRP) injections — a treatment that concentrates the body’s own healing factors from a small sample of your blood and delivers them to the injured tendon. One critical factor determines success: timing. Non-surgical treatment with PRP and casting works best when we see the patient early, before the torn ends of the tendon retract up the leg. Once the gap widens, the tendon is far harder to heal without surgery.

- Your activity level and goals — high-demand athletes may benefit from surgical repair

Recovery
Whether treated with PRP and casting or with surgery, recovery is a gradual, protected progression — immobilization, then a walking boot with heel lifts, then physical therapy to rebuild calf strength and restore push-off. Returning to sport too soon risks re-rupture, so we guide each milestone carefully.
