When NFL wide receiver Pierre Garcon injured his foot on an 88-yard touchdown and then struggled for weeks afterward — X-rays showing no break, yet the foot “less than 100 percent” — it was a textbook example of a frustrating and often misunderstood injury: turf toe. His case is a useful way to explain a problem we treat regularly in athletes and active patients.
What Is Turf Toe?
Turf toe is a sprain of the big-toe joint — a stretch or tear of the ligaments and soft tissues beneath the joint — that happens when the big toe is forcibly bent upward, often when pushing off on a hard or artificial surface. Because it is a soft-tissue injury, an X-ray frequently looks normal even when the toe is significantly damaged, which is why athletes are sometimes cleared too soon and then can’t regain their explosiveness.
Symptoms
- Pain, swelling, and stiffness at the base of the big toe
- Pain that worsens when pushing off or bending the toe upward
- A weak or unstable feeling in the toe and reduced power on that foot
Why It Shouldn’t Be Rushed
Turf toe is graded by severity, and the right diagnosis matters. We evaluate the joint with a hands-on exam and diagnostic ultrasound or MRI when needed, because a missed high-grade turf toe can lead to lingering pain, stiffness, and even arthritis of the joint down the road.
Treatment
- Rest, ice, compression, and elevation in the acute phase
- A stiff-soled shoe, turf-toe plate, or taping to limit painful motion
- A short period of immobilization for more severe sprains
- Physical therapy to restore strength and motion before return to sport
- Surgery only for the most severe tears
If you have nagging big-toe pain after a push-off injury, have it properly evaluated rather than playing through it.
