A hammertoe is a contracture of one of the smaller toes caused by a muscle imbalance between the tendons on top of the toe and those underneath. The toe buckles at its middle joint, pointing the knuckle up into the shoe and the tip down toward the ground. Early on the deformity is flexible — you can straighten it with your fingers. Over time it becomes rigid and fixed. Along the way, friction against the shoe builds a painful corn on top of the toe, and the retracted toe drives the metatarsal bone behind it downward, creating a stubborn callus on the ball of the foot.
Preventative and corrective (flexible hammertoes). Custom functional orthotics — think of them as prescription contact lenses for your feet — correct the abnormal foot mechanics that create the muscle imbalance in the first place. Used early, orthotics can allow flexible hammertoes to straighten over time. Calf stretching helps rebalance the pull on the toes. This is the window where the problem can genuinely be reversed without surgery.
Symptomatic relief. Roomier toe boxes, hammertoe pads, and professional corn care keep you comfortable. A caution from our surgeons: over-the-counter corn removers are acids that burn tissue — never use them between the toes, and never use them at all if you have diabetes or poor circulation.
Surgical correction. For a flexible hammertoe, a minimally invasive tendon release — often a single stitch, performed in-office under local anesthesia — offers rapid recovery. For rigid hammertoes, we remove a small section of bone at the stiff joint and, in severe cases, stabilize the toe with a pin or internal implant or fuse the joint. Most patients walk the same day in a surgical shoe and return to a stiff-soled walking shoe around two weeks. Temporary swelling and a “floppy” feeling in the toe for a few weeks are normal parts of healing. We perform these procedures in-office or at an outpatient surgery center with sedation — whichever suits your case and comfort.
