Hallux rigidus is arthritis at the base of the big toe. This causes stiffness and pain at the joint. In the long run, it can get harder to bend the toe.
This is a form of degenerative arthritis which means it results from wear and tear. Aside from stiffness and pain, hallux rigidus can result to swelling and inflammation around the joint. A bony bump may also develop on top of the joint.
Diagnosis can be made by physical examination. Xrays can identify the extent of joint regeneration and the size of bone spurs. Rarely, MRI and CT scans are needed.
For big toe joint pain, rest, ice, and over the counter painkillers or stronger medications prescribed by the physician can help relieve pain and swelling. In cases of very painful toes, steroids injected into the joint can reduce the inflammation. If conservative treatment does not help and pain and stiffness are limiting the normal daily activities, then surgery should be considered.
Symptoms of Hallux Rigidus
- Pain and stiffness in the big toe when you walk, stand, or push off
- Pain that worsens in cold or damp weather
- Difficulty bending the toe upward (dorsiflexion)
- Swelling and inflammation around the joint
- A bony bump on top of the joint that rubs against shoes
- Limping or shifting weight to the outside of the foot to avoid the toe
Symptoms often begin as occasional discomfort — a milder, earlier stage called hallux limitus — and progress to constant pain as the cartilage wears down.
What Causes Big Toe Arthritis?
Hallux rigidus develops when the cartilage in the first metatarsophalangeal (MTP) joint wears out. Common contributors include:
- Abnormal foot mechanics, such as a long or elevated first metatarsal
- Prior injury to the big toe, including a stubbed or “turf” toe and old fractures
- Repetitive stress from sports, work, or activities that bend the toe
- A family history of foot problems or inflammatory arthritis such as gout or rheumatoid arthritis
Non-Surgical Treatment Options
Most patients improve without surgery, especially when the condition is caught early. At Family Foot & Leg Center we typically begin with:

- Anti-inflammatory medication and activity modification

Surgical Options When Pain Persists
When conservative care no longer controls the pain, several procedures can help, chosen based on how much arthritis is present:
- Cheilectomy — removing the bone spur and a portion of the joint to restore motion in mild to moderate cases
- Joint-preserving osteotomy — realigning the bone to relieve pressure on the joint
- Cartilage or implant procedures — resurfacing the joint to maintain movement
- Joint fusion (arthrodesis) — the most reliable, long-lasting solution for severe, end-stage arthritis
