Flat feet are not always a problem — but when the arch collapses and causes pain, instability, or progressive deformity, treatment matters. Adult-acquired flatfoot most often develops when the posterior tibial tendon, which supports the arch, weakens and fails over time. The video below walks through how we approach surgical correction.
When Is Surgery Considered?
Most flat feet are managed without surgery — with custom orthotics, bracing, supportive shoes, calf stretching, and anti-inflammatory care. Surgery enters the conversation when those measures no longer control the pain, when the deformity is progressing, or when tendons and ligaments have torn. The goal is to relieve pain, rebuild the arch, and restore a stable, functional foot.
Common Flatfoot Procedures
- Tendon repair or transfer to restore the failed posterior tibial tendon
- Osteotomies — realigning the heel and midfoot bones to rebuild the arch
- Minimally invasive implants such as a subtalar stent (HyProCure) for appropriate candidates, which can mean a faster recovery
- Joint fusion (arthrodesis) for rigid, end-stage deformities
The right procedure depends on whether the flatfoot is flexible or rigid, how severe it is, and your activity level and health — which is why an individualized plan is essential.
Why Experience Matters
Flatfoot reconstruction is highly specialized. Choose a board-certified reconstructive rearfoot and ankle surgeon who performs these procedures regularly. If you have collapsing arches and pain that limits your day, an evaluation will lay out your full range of options.
