They are actually very different conditions — and telling them apart changes the entire treatment plan. That is why a podiatrist and an X-ray are so important.
Bunion vs. Hallux Rigidus
A bunion (hallux abductovalgus) is a malalignment: the first metatarsal drifts inward and the big toe leans toward the others, pushing the joint out into a visible bump. On X-ray, the joint space is preserved — the cartilage is still there.
Hallux rigidus is arthritis of that same joint. On X-ray the joint space is narrowed or gone, with bone spurs and bone-on-bone contact. The bump here is often arthritic bone on top of the joint rather than a sideways lean.
In the X-ray of a single patient below, the arrows show a normal joint space on one side (a bunion) and no joint space on the other (arthritis). They look identical on the surface, but the imaging tells the real story.
Why the Difference Matters for Treatment
- True bunion: corrected by realigning the bone — often with a minimally invasive, scarless bunionectomy.
- Mild/moderate arthritis without severe angulation: a joint implant or joint-preserving procedure may restore motion.
- Severe arthritis with significant angulation: fusion of the joint generally gives the most reliable, long-lasting relief.
Treating a bunion as if it were arthritis — or vice versa — leads to disappointing results, so an accurate diagnosis comes first.
