As a podiatrist here in Port Charlotte, I have the privilege of treating a diverse community of athletes – from weekend warriors enjoying our local tennis courts to competitive runners and student-athletes. While the Florida sun keeps us active year-round, the constant demand we place on our lower extremities often leads to a specific set of challenges.

In my practice at Family Foot & Leg Center, I often tell my patients that the foot and ankle are the “foundation” of the house. If the foundation is cracked or unstable, the rest of the structure suffers. Whether you are a marathoner or a pickleball enthusiast, understanding the most common injuries can be the difference between a quick recovery and a season on the sidelines.

Here are the top foot and leg injuries I see in athletes, and what you need to know to keep moving.

The Morning Nemesis: Plantar Fasciitis

By far the most common cause of heel pain, plantar fasciitis, involves inflammation of the thick band of tissue (the fascia) that connects your heel bone to your toes. Athletes in high-impact sports or those who have recently increased their mileage are particularly susceptible.

The Symptom: A sharp, stabbing pain int the heel, usually worst with the first few steps in the morning.

The Podiatrist’s Perspective: We often find that poor biomechanics – like overpronation or flat feet – are the root cause. While “resting” is a tough pill for athletes to swallow, we focus on stretching the calf muscles and utilizing custom orthotics to redistribute pressure.

The Overworked Anchor: Achilles Tendonitis

The Achilles tendon is the largest and strongest tendon in the body, but it isn’t invincible. It absorbs forces up to 12 times your body weight during running. Achilles tendonitis occurs when this tendon becomes irritated through repetitive stress or sudden increases in intensity.

The Danger Zone: If you ignore a dull ache at the back of your leg, you risk developing tendinosis (chronic degeneration) or, worse, a full rupture.

Treatment: We emphasize the “10% Rule” – never increase your training volume by more than 10% per week. In the clinic, we may use Shockwave Therapy or physical therapy to stimulate healing in the tendon fibers.

The Sudden Snap: Lateral Ankle Sprains

Ankle sprains are the most common acute injury in sports like basketball and soccer. They occur when the foot “rolls” inward, stretching or tearing the ligaments on the outside of the ankle.

Why You Shouldn’t “Walk It Off”: Up to 30% of people who suffer a sprain will experience recurrent injury. This leads to Chronic Ankle Instability.

The Dr. Singh Approach: I perform a thorough evaluation to rule out small “avulsion” fractures. Proper bracing and “proprioception” (balance) training are essential to retraining the ankle to stay stable on uneven surfaces.

The Hidden Cracks: Stress Fractures

Unlike a clean break from a fall, a stress fracture is a tiny hairline crack in the bone caused by repetitive “submaximal” loading. We see these most often in the metatarsals (the long bones of the foot) or the tibia (shin bone).

Who is at Risk? Runners who switch from a treadmill to concrete, or those with nutritional deficiencies (low Vitamin D or Calcium).

Diagnosis: Interestingly, stress fractures often don’t show up on X-rays for several weeks. If I suspect one, I may order an MRI or a bone scan for an early, accurate diagnosis to prevent the crack from becoming a complete break.

The Persistent Burn: Shin Splints (MTSS)

Medial Tibial Stress Syndrome, or shin splints, is the bane of many high school track stars and new runners. It is an inflammation of the muscles, tendons, and bone tissue around your tibia.

The Fix: This is often a footwear and surface issue. I often perform a gait analysis to see if the athlete is rolling their foot inward too much, which puts excessive torque on the shin. Replacing shoes every 300-500 miles is non-negotiable for prevention.

When to See a Specialist

Many athletes try to “power through” the pain, but in podiatry, early intervention is the key to longevity. If you notice swelling that doesn’t go down with ice, pain that changes your gait (how you walk), or numbness and tingling, it’s time to seek a professional evaluation.
